    Hello, we have had an employee on leave for over 1 year now. She was declined for LTD and sued our carrier and that has now been settle (for privacy reasons, we don’t know how). Monthly she sends in a note from her nurse practitioner advising that she is not well enough to return to work that but that’s it. We are looking for a prognosis for return to work. The nurse practitioner’s office told me our employee that I had to have her sign a “consent to release information” form in order for them to provide me with any info. Do you know where I can find something like that? Thanks. Kristy McDougall

    We should have such a form. Have you tried searching the HRI site? Also, if you tell me which jurisdiction you’re in, I’ll see if I can track down a template directly from the Privacy Commissioner Office. Thanks.
