Automated External Defibrillator (AED) Incident Report Form

Here’s a template of an incident report form that you should complete any time you use your AED in a first aid emergency situation. Have the person who used the AED complete and submit the form within 48 hours of the incident. The form comes from the Canadian Red Cross. But be sure to make whatever changes you deem necessary to adapt it to your own needs and circumstances.
