MODEL FORMS FOR REPORTING INCIDENTS OF VIOLENCE

Employers have a legal duty to take measures to prevent workplace violence. In many jurisdictions, these measures include adopting a workplace violence prevention program. Procedures for employees to report incidents or threats of violence is a central elements of such programs. Here are 3 Model Forms for reporting that you can adapt:

  • Form A can be used to report general incidents or threats of violence
  • Form B can be used to report incidents of abuse, violence or harassment of an employee by a supervisor
  • Form C can be used to report threats of domestic violence
